For years, patients have depended on me to provide this valuable service. I perform routine Well Adult and Child examinations as well as Sports Pre-participation and College Pre-entrance examinations. I still believe a thorough history and physical examination is helpful even in this age of advanced medical technology.

Vaccinating A Child

IMMUNIZATIONS

Immunizations are an important part of preventative medicine. I provide all the routine scheduled vaccines recommended for newborns and children to pre-teens and teenagers, as well as the vaccines recommend for adults and seniors. In addition, I can provide travel vaccines for those traveling outside the United States.

For Well Patient Physical Examination visits, I order blood tests to screen for endocrine, blood, liver, cardiac, and kidney disease. For other visits, I may order specific blood tests for diagnostic purposes or to help manage your specific chronic health conditions. Blood draws can be done at my clinic or if you prefer, at any outside laboratory facilities such as Quest Diagnostics, LabCorp, or El Camino Hospital.

APPOINTMENTS

Please call my office if you have a medical problem that needs evaluation and/or treatment. As a Family Physician, I have the skills and training to handle the most common medical issues.
In life-threatening situations, such as a heart attack or traumatic injury, dial 911 for fast paramedic assistance, or go directly to a hospital emergency room.

URGENT & WALK-IN APPOINTMENTS

Urgent appointments are for acute problems that may need to be evaluated and/or treated within 24 hours. Please call my office first to determine your need to be seen that day. If you do need to be seen, we will fit you into the day's schedule.

We will try to work in all walk-in patients who do not have a scheduled appointment into the day's schedule if time is available.
 

CANCELLATION POLICY

If you are unable to keep your scheduled appointment, please call to cancel your appointment so someone else can be seen for their medical needs. There is a $25.00 fee for any missed 15 to 30 minute appointment and a $50.00 fee for any missed 60 minute appointment not previously cancelled at least 24 hours prior to the appointment. You can call the office at (408) 863-0709 at any time to cancel your appointment.

FINANCIAL POLICIES

I am a preferred provider for most of the major insurance companies. Please call my office for the most up to date list and to verify your coverage. If you do not have insurance, you can pay at a discounted rate at the time of your service. In addition, I have an very affordable Direct Primary Care program which may help those with no medical insurance or those with a high deductible medical plan. Please see the Direct Primary Care page for more details about the program.

PAYMENT OBLIGATION

Payment for services is expected at the time of service unless prior arrangements have been made. Deductibles and copayments should be made at the time of service. You may pay by cash or credit card. As a courtesy, the office will file insurance claims for services rendered, however, the final responsibility for payment will be yours.

BILLING STATEMENT

Billing statements are sent out regularly. They will include the itemized service fees and payments received from you or your insurance company. All charges should be paid within 15 days from the billing date in order to avoid late charges. You may pay by check or credit card.

DIRECT PRIMARY CARE

CONCEPT

Direct Primary Care (DPC) is a health care model that restores a personal relationship with your physician. The goal is to simplify the finances in order to provide more time with your physician, more personal care, and to place the incentive on keeping you healthy rather than on paperwork or getting paid. This is accomplished by removing insurance companies from the relationship between the patient and primary care physician.
The idea is to use health insurance only for unexpected and expensive problems. This is similar to how people commonly use home or car insurance. Though you would not use home insurance for a leak under your sink, you would if a tree fell on your home unexpectedly. A DPC practice can take care of 80-90% of your health care needs for a fraction of the cost that an insurance-based medical practice could. Insurance is then available for the unexpected catastrophic events.
In short, Direct Primary Care removes the insurance company from the middle of the relationship between you and your doctor. With your doctor focusing on you, you get better care. Patients utilizing a Direct Primary Care practice in combination with a high deductible insurance plan stand to get higher quality personalized health care while saving money.
My Direct Primary Care Program will provide you with comprehensive primary and preventive care in my office as well as telephonic and e-mail access for one low monthly fee. Please be aware that this is not health insurance. The only services provided for the monthly fee are services I provide as your primary care provider in my office. For your monthly fee, you can visit my office as frequently as you need. If there is a matter that can be handled on the phone or by e-mail, we can do that in lieu of an office visit.

  • No enrollment fee.

  • 1 year service contract required. Cancellations within the first year will be subject to an early cancellation fee equal to the number of months remaining on the 1 year contract. At the end of your one year contract, you will be on a month-to-month contract and there are no cancellation fees if you leave the program.

  • If you leave the program and then re-enroll at a later date, you will be required to sign a new one-year contract.

  • Monthly fees are as follows:

Family discounts are available. The oldest parent will be charged full price, and children and spouses will receive at 10% discount off monthly fees.

BACKGROUND

Dr. Chiu grew up in Santa Clara County and graduated from Los Gatos High School. At the University of California, Davis, he received a B.S. degree in Zoology, graduating Magna Cum Laude. He was also elected membership in Phi Beta Kappa and Phi Kappa Phi honor societies. In 1990, he graduated from the University of Southern California, School of Medicine which was followed by a three year residency in Family Practice at Kaiser Permanente, Los Angeles. Dr. Chiu has a special interest in sports medicine and has been the family physician for several professional and nonprofessional sports teams in the South Bay Area. Dr. Chiu enjoys spending his free time with his wife and two daughters and pursuing his hobbies of gardening, woodworking, and playing the piano.
